4668.0200 CLASS C PROVIDER, INDIVIDUAL PARAPROFESSIONALS.

Subpart 1.

Scope.

This part applies only to a paraprofessional with a class C license under part 4668.0012, subpart 3.

Subp. 2.

Services.

The licensee may perform:

A.

home health aide tasks;

B.

home care aide tasks; and

C.

home management tasks.

Subp. 3.

Training.

The licensee who performs home health aide tasks or home care aide tasks must meet the requirements of part 4668.0130 before a license will be issued.

Subp. 4.

Record of supervision.

The licensee who performs home health aide tasks must maintain a record of the supervision required by part 4668.0100, subpart 9.

Subp. 5.

Records.

The licensee must maintain a written record of the services provided at each visit to clients.

Subp. 6.

Notice of clientele.

Upon request of the commissioner, class C licensees shall provide the name, address, and telephone numbers of all or specified clients and the clients' responsible persons.
