A person seeking licensure to practice dental hygiene must present an application and credentials as determined by the board and meet the following requirements of the board.
The applicant must furnish certification of having passed the national board examination as defined in part 3100.0100, subpart 15.
The applicant shall submit evidence of satisfactorily passing a board approved examination designed to determine the applicant's level of clinical skills.
The applicant shall furnish satisfactory evidence of having been granted a diploma or certificate in dental hygiene from a school accredited by the Commission on Dental Accreditation.
For identification purposes, the applicant shall furnish one notarized unmounted passport-type photograph, three inches by three inches, taken not more than six months before the date of the application.
The applicant shall furnish evidence of good moral character satisfactory to the board and certification from the Board of Dental Examiners in the state or Canadian province in which the applicant is already licensed.
