The requirements of subparts 2 to 4 shall be met in order to renew a license.
All licenses expire on the last day of the month of initial licensure of the year due and each licensee is responsible for renewing the licensee's license. A salon license that is not renewed by the last day of the month of initial licensure of the year due is considered unlicensed as of the first day of the following month. Not receiving a notice of renewal from the board does not constitute a valid excuse for not renewing the license.
The licensee shall pay the fees identified in Minnesota Statutes, section 155A.25, before the expiration of the current license.
The licensee shall state the name, address, and telephone number of the salon owner on the renewal application.
