services for vulnerable adults and persons with developmental disabilities not
receiving home and community based waiver services, specifying eligibility for
the services and included activities; requiring and providing for development
and review of personal service plans, specifying certain plan content
requirements; specifying certain county or contracted vendor qualification
requirements for targeted case management services reimbursement eligibility
purposes and providing for payment for the services under medical assistance and
the MinnesotaCare program, specifying certain county financial responsibility,
authorizing the commissioner of human services to suspend, reduce or terminate
reimbursements to providers failing to meet reporting or other requirements;
requiring the commissioner in consultation with county boards to establish the
program and performance measures to evaluate the effectiveness of the services
