Providing for release of certain birth information to adopted persons; providing
for release by the commissioner of health of copies of original birth
certificates of adopted persons or disclosure consents or affidavits of birth
parents and eliminating the requirement for the commissioner to notify the
commissioner of human services of requests for the information; authorizing
parents identified on original birth certificates to file with the commissioner
of health an affidavit objecting to the release of information relating to
parent identity, authorizing adopted persons to petition the court for release
of the information and parents to revoke affidavits by filing a consent to
disclosure form with the commissioner; specifying certain requirements and
restrictions for release of information by the commissioner; requiring release
of requested information after death of the parent; requiring the commissioner
to inform the public and adoption agencies and agencies to inform clients of the
provisions; repealing certain search requirements of the commissioner of human
