Requiring the commissioner of health to establish a planning and transition
grant program for eligible providers of older adult services, defining eligible
provider of older adult services and specifying authorized uses of the grants;
requiring the commissioner to establish a grant review panel to make
recommendations on grant awards, specifying membership requirements; providing
for allocation of the grants by the commissioner, requiring provider application
to the panel and signing of a grant contract, specifying certain panel and
commissioner consideration requirements and a commissioner final decision time
and maximum grant limits; requiring the commissioner to evaluate the
effectiveness of the grant program and appropriating money to the commissioner
