commissioner of health by physicians and facilities performing abortions on
minors and certain incompetent women, requiring the commissioner to prepare and
distribute the reporting forms; imposing a late fee for failure to report;
requiring the commissioner to annually issue a public report providing
statistics compiled from the reports; authorizing certain modifications to the
requirements by the commissioner to achieve administrative convenience or fiscal
savings; authorizing citizen suits for failure of the commissioner to issue the
public reports, subjecting the commissioner to sanctions for civil contempt
under certain conditions; providing for the awarding of attorney fees in certain
